希望上一篇文章的慘痛經歷不要再度發生, 在此條列一下正確的健檢步驟:

Page 1-2: Instructions
Page 3-4: Please fill in your personal information and sign at the bottom.

Page 5:
Item 1 (MMR) & Item 3 (Td) :
Go to Heping Hospital. Remember to bring your photo and 疫苗卡 along.
Charge: around TWD 650.

Item 2 (Hepatitis B series of 3) & Item 5(testing for chicken pox) :
Required for HST students.

Item 4 (Meningococcal) :
Go to National Taiwan University Hospital
Charge: TWD 1,486
Note: This vaccination is optional. If you choose not to have this vaccination, sign on Page 9-10.

Page 6:
Since Taiwan is on the list, we need to have Mantoux PPD test.
Go to Branch for Disease Control and Prevention.
Charge: kinda forgot.. i'll update it after confirmation.
Important Note: The hospital provides the test only on Wednesday afternoons.
And you have to go back to have the doctor check your test results on the following Saturday. Be sure to save time for this.
Don't forget to bring a copy of your passport on that Saturday morning.

Page 7-8:
Doctors in Heping Hospital will help you fill in the information required.
Be sure to review the first section in Page 7 (History and Review of Systems) and check yes or no.

Page 9-10:
If you decide not to accept the vaccination as stated in tem 4 (Page 5), you need to sign on this form.
